Description:
Responsibilities 1.You will be assessing the initial screening and establishing the client's eligibility for support under the Disabled Persons Act. 2.You will be partnering with your client developing a Transition Pathway Plan which identifies the needs and aspirations of the young person and takes account of social care, health and education issues and guidance in conjunction with the young person and/or their carers. 3.You will be providing and assisting in educating and counselling young people with statements of special educational needs who are permanently and substantially disabled. 4.You will accountable of the maintenance support of a database of all children and young people with statements of special educational needs who are substantially and permanently disabled. 5.To carry out, where appropriate, complex single or multidisciplinary assessments to meet the identified needs of the young person in conjunction with them, their carers and relevant professionals. 6.Together with your young clients you will asses their careers and produce a care plan advising them on the existing market place in order to obtain the most appropriate and cost effective supplier of services consistent with the clients need ensuring that the agreed care plan can be provided within the available budget. 7.Where appropriate you will negotiate one-off contracts with prospective service providers. This will require liaison with the Service Development Officers to ensure that such contracts and specifications are appropriate. 8.You will also monitor the care being provided to ensure that needs are continuing to be met and continually re-assessing the needs of a young person p and, when appropriate, you will modify the care plan following consultations with them and/or their careers. 9. You will be responsible for providing on-going support and advice to young people and their careers as well as identifying areas of need that cannot be met and escalated to a Team Manager when needed. 10.Maintaining all necessary records and documentation in line with council administration procedures and use computerised information systems. 11.You may be responsible for the supervision of a member of staff within the team depending on experience.
Qualifications:
Buckinghamshire Country Council - Social Services Department Position: Youth Transition Worker Location: Buckinghamshire, UK Job Description Objectives: You will be responsible to support young people and their carers through the process of transitions, following the ranges of 14+ Review through to adult services at 18+. To ensure that appropriate ranges of services are identified in their Transition Path Plan. You will also be assisting with their transition plans reviewing them annually, their careers and goals. Requirements: Hold a Bachelor Degree or Masters in Social Work or equivalent (BSW or MSW) Be eligible to work in the UK Minimum of 1-2 years of experience working with youth of children in a similar capacity: Career Counselling, Cased Load Assessment.
